Salary: $29.12/hour +

Harbourview Family Health Team (HFHT) is a well-established primary care team based in Thunder Bay, dedicated to delivering high-quality, team-based care to our patients and their families. Our interdisciplinary team includes nurse practitioners (NP), registered nurses, registered dietitians, registered practical nurses, social workers, administrative professionals, and 14 family physicians.

Job Summary
RPNs work to their full scope of practice within a pod model, supporting physicians and NPs in the delivery of routine, preventive, and chronic disease care. This role focuses on proactive follow-up, health maintenance, and team-based care coordination.
Key Responsibilities
- Conduct patient assessments and document findings within RPN scope
- Provide routine nursing care (e.g., vital signs, immunizations, injections, wound care, ear irrigation, specimen collection)
- Support chronic disease management (e.g., INR monitoring, diabetes, hypertension, COPD, asthma, heart failure, osteoporosis)
- Assist with preventive care initiatives including immunizations and screenings
- Provide nursing support to providers in selected procedures when required
- Monitor patient status and communicate changes to the physician or NP
- Assist with medical administration such as reviewing inboxes, faxes, forms and prescription renewals
- Support transitions of care, hospital follow-ups, referrals, and diagnostic testing
- Maintain accurate and timely EMR documentation
- Assist in the new patient onboarding process
- Other duties as assigned within the RPN scope of practice
- Participate in pod huddles, team meetings, and quality improvement initiatives
- Contribute to a collaborative, respectful, and learning-focused workplace
Professional & Practice Expectations
- Adhere to CNO standards, HFHT policies, medical directives, and IPAC requirements
- Use evidence-based practices and support quality improvement initiatives
- Engage patients using a trauma-informed and culturally safe approach

Qualifications:
- Registered Practical Nurse, licensed with the College of Nurses of Ontario (in good standing)
- Recent related experience: preference for two years of related clinical experience preferred (community or hospital setting)
- Current CPR certification and professional liability insurance
- Experience working in an interdisciplinary team
- Comfortable using EMRs and clinical technology
