Harbourview Family Health Team (HFHT) is a well-established primary care team based in Thunder Bay, dedicated to delivering high-quality, team-based care to our patients and their families. Our interdisciplinary team includes nurse practitioners, registered nurses, a registered dietitian, social workers, administrative professionals, and 14 family physicians.

Job Summary:

We are seeking qualified applicants to lead our clinical programs and our allied health team to delivery comprehensive, innovative, and high quality primary care with the right provider at the right time.

As a key member of our leadership team, our Clinical Program Manager will lead, coordinate, and collaborate with our team to develop, manage, and monitor clinical service delivery. Working to their full scope of practice, the successful candidate will have a clinical degree and be registered with their college, have expertise in the areas of program development and management, have demonstrated experience in integrating evidence-based design into practice, and will be a supportive leader who can foster a health organizational culture.

Key Responsibilities:

  • Lead the development and monitoring of clinical performance metrics and outcomes
  • Work collaboratively with interdisciplinary team members to support patient care planning and coordination
  • Oversee the allied clinical team, managing scheduling and manage clinic coverage to maintain seamless are
  • Integrate evidence-based programming and practices into clinical programs, enhancing the quality of care for patients
  • Support clinical program delivery research projects
  • Contribute to strategic direction of Harbourview Family Health Team both internally and within the greater context of primary care services in Thunder Bay
  • Guide professional development initiatives and evaluate continuing education requests
  • Manage vaccines and immunizations, ensuring safety and proper storage
  • Maintain and update medical directives, policies, procedures.
  • Foster communication across teams, and promote
  • Coordinate clinical student placements, including onboarding
  • Maintain professional relationships with service providers and related organizations
  • Ensure confidentiality and exercise appropriate discretion when handling sensitive client and personnel information
  • Follow all organizational policies and procedures to maintain a safe work environment
  • Attend required training sessions and comply with all health and safety standards, including WHMIS
  • Other duties as required

Work Environment:

Reporting to the Executive Director, the position is in-person and works in close partnership with the leadership team, including the lead physicians. The Clinical Program Manager is an integral part of our service delivery, overseeing clinical programming and ensuring patients receive the right care at the right time while also supporting a positive and innovative organizational culture.

Qualifications:

  • Experience in leading teams or formal leadership position – 2 years
  • Current certification of registration with a registered college for clinical practice
  • Masters in health care administration, business, or other health-related field – preferred
  • Certifications in project management, lean six-sigma, leadership, and inclusivity – preferred
  • Current licensure as a clinical health care provider
  • Demonstrated experience in a health care, primary care or family health team setting
  • Proficient in evidence-based practices for primary care delivery, with a focus on health promotion, disease prevention, and chronic disease management
  • Clinical and assessment skills, with demonstrated competence in collaboration, critical thinking, and innovative problem-solving
  • Excellent written and oral communication skills
  • Strong organizational and time-management abilities
  • Effective both independently and as part of an interdisciplinary team
  • Proven ability to take initiative and contribute to team activities, organizational development, and quality improvement initiatives
  • Demonstrates leadership within the clinical team and a commitment to supporting a collaborative, team-based care model
  • Bilingualism (English/French or other languages) is an asset
