About Us

The Harbourview Family Health Team offers high quality primary health care for its patients.  An inter-professional group of health care practitioners, including family physicians, provide health care in an integrated, respectful and holistic environment.  The focus of the family health team is on chronic disease management and prevention as well as health promotion.  By working collaboratively, the team is able to provide a broad spectrum of Ministry of Health funded, primary health care services to its patients to ensure they get the care they need when they need it.

Family Health Teams are an important part of changing both the health care system and the way people think about health care.  Family Health Teams focus on illness prevention and health promotion.  The teams looks at the best ways of managing chronic illnesses such as hypertension, diabetes and depression.  In doing so, they are able to provide care that is more accessible and comprehensive and gives patients the

opportunity to be more involved in making decisions about their own health.

Family Physician

Family physicians, through education and residency training, possess distinct attitudes, skills, and knowledge which qualify them to provide continuing and comprehensive medical care, health maintenance and preventive services to each member of the family regardless of sex, age, or type of problem, be it biological, behavioral, or social. These specialists, because of their background and interactions with the family, are best qualified to serve as each patient’s advocate in all health-related matters, including the appropriate use of consultants, health services, and community resources.

Nurse Practitioner

A nurse practitioner (NP) is a registered nurse with advanced university education who provides personalized, quality health care to patients. Nurse practitioners provide a full range of health care services to individuals and families within our clinic. We work in partnership our Family Health Team of physicians, nurses and other health care professionals such as social workers, midwives, mental health professionals and pharmacists to keep you and your family healthy.

Registered Nurse

The Registered Nurses within the Harbourview Family Health Team work with our doctors, social workers, dietitians, and pharmacists to provide care that promotes health and prevents disease. Nurses at the Harbourview Family Health Team work with patients to assist with the management and prevention of chronic disease through patient centred care. They assess patients’ needs and work with them to develop a plan of individual care emphasizing the patients’ central role in managing their illness.This work complements traditional patient education by promoting good lifestyle management to support healthy outcomes and helps patients achieve the best quality of life with their chronic condition.

Social Worker

Social workers at the Harbourview Family Health Team use a strengths-based approach in their work to help individuals learn new ways to respond to stressful events in their lives. Social workers can help patients develop skills and enhance coping capacities so that overall health is achieved and positive changes can be made.

Once a referral is received from your primary health provider, an initial assessment is made of the patient’s psychosocial functioning and interventions are suggested as needed. These could include one to one counselling, group education and/or a referral or linkage to other community resources and supports.

Social Workers at the Harbourview Family Health Team are registered members of the Ontario College of Social Workers and Social Service Workers and are also members of the Ontario Association of Social Workers.

Registered Dietitians

Registered Dietitians are regulated health care professionals who are trained to give advice and provide education about food and nutrition in a variety of settings.  Dietitians help people to make healthy food and lifestyle choices that impact their overall health and disease management.

At the Harbourview Family Health Team, our Registered Dietitian works with the Physicians, Nurse Practitioners, Registered Nurses and Social Workers to help patients achieve their best health.  Nutrition counselling is provided for individuals, families and in group settings. Whether you would like to learn about meal planning for a specific health condition or simply want to improve your diet, ask about a referral to the dietitian.
